During the latter half of the Undead War the Necromancer known only as Nagash the Mighty reigned supreme within his fortress called Cripple Peak built on the foundations of a massive Warpstone Meteor. He and the nearby Skaven clans quickly went to war over it. The battle raged on for years until Nagash was assassinated, leaving the Skaven to claim Cripple Peak. Nagash's knowledge of undeath was however too great and he would return half a century later to destroy any clans that had settled within. However by this time the Skaven had already mined most of the Warpstone, greatly diminishing his power; and with their combined efforts the skaven were able to defeat Nagash. However in his last moments he released a dark plague which the Skaven dubbed The Necromancers plague.

Skaven legends say that when plague ravages a region, a spirit will sometimes walk its lands. These deathrisen are believed to be the long-term effects of Nagash's final act of revenge upon the living. first of these creatures were known as Lurea'ag or “Ladies of the Hollow” but they’re more commonly known today as plague maidens these are some of the most insidious deathrisen a traveler may have the misfortune to come across. These deathrisen would walk the from town to town resembling deathly pale women and dressed in a peasants gown and covered in a hooded shroud.

These villages already fearing the spread of this plague would often deny them entry and they would then pass on to the next nearby town. Except every so often there is always one kind stranger whom she manages to convince to give her a ride into the town. That night, they would roam the village leaving corpse-strewn streets with rats and bugs scurrying about all around in their wake. However, as their plague wreaked havoc on the town, in recompense for their actions the Plague Maiden remembers the kind strangers house and allows them to survive the plague.

Whether this was through being saved by virtue of their act of kindness or just so that someone would live to tell the tale is unknown. They delight in the slow suffering of mortal beings. So it's likely this is less an act of mercy and more of a professional courtesy. No one knows whether this spirit brings the pox with them or if they're merely drawn to it like a moth to a light. The Elder plague maidens were once the keepers and Oracles of what was once known as the Dolman Alters. Before being hunted down and killed these elders Maidens referred to as “Mothers” would pass on their teachings to younger plague maiden “daughters” as they rise from death and return to the void, repeating this cycle in symbiosis with the recurrence of plagues among the living.

As the name suggests, these wraiths take the appearance of females, though exactly why this is remains a mystery. Technically speaking given the transformative nature of deathrisen ascension there is nothing to suggest they couldn't just as easily been male spirits before dying and being risen as Plague Maidens. Regardless of their creation, they’re feminine prowess evolved over time; the moor of death's seductive call beyond the graveyard’s gate and their song is the hearts darkest desire. They Posess a direct connection with the Necromyc study of plagues, and their notable similarities to the Dark Elder Khal'Rhun'Kannon's transition to Grimm and her tears over the loss of the primes and her sacrifice to the Kuust may have been the birth of this undead strain. They arise from powerful negative emotional charge of painful long-suffering illness and death plague-ridden communities. It’s believed they survive by feeding off the hopes and fears of the living within these townships.

They were commonly known to have fewer overt weaknesses compared to other wraiths such as being bound by some form of unfinished business or some duty bound ties to the material planes; but they are still vulnerable to the same strikes from a cold iron blade just the same as any other incorporeal being. They have the ability to summon waves, plague-ridden insects and rats that seek out their foes. This does however mean close quarters combat would run the risk of being swarmed by these plague-ridden vermin. They undoubtedly pose a great danger to the immune system, so a well prepared fighter should take heed to always bring the proper tools and take precautionary measures to prevent catching or spreading the contagious illnesses they carry should you find yourself in an encounter with one.
